Your membership card contains savings in the form of manufacturer’s rebates, product and service discounts and purchasing points. You can save money all three ways:

• Participating Providers have negotiated with select manufacturers and suppliers to save their patients and/or customers on select purchases.
• Participating Providers also offer discounts on select services and products provided.
• Each time you use your Card you begin building points toward other purchases you plan to make in the future.

USING THE CARD

1. You are eligible for all benefits on the card for a full 12 months. You will receive a renewal notice by email prior to the expiration date each year.
2. To access the benefits on your card go to the Participating Provider location for the services or products you want and have your card scanned. A benefit list will be printed for you.
3. Take your membership card with you to your appointments so it can be used to process your benefits.
